About The Position

This is a grant funded position. The Oklahoma City Housing Authority receives annual funding from Oklahoma Department of Mental Health and Substance Abuse Services (ODMHSAS) to provide sober living services to residents living at Providence Apartments. Scope: Responsible for coordinating care and case plans for residents, participating as a member of the referral approval team or assigned team lead at the ODMHSAS contracted provider, administering onsite intake process, and assisting with required program certifications for the Sober Living Program at Providence Apartments. Accountability: Directly responsible to Assistant Director of Resident Services. Relevant duties include but not limited to: Providing case management and coaching using a client-centered approach to residents. Working collaboratively with residents to holistically meet resident’s needs, creating and maintaining a confidential environment, coordinating management of assessment tools, and assist in coordinating educational and/or community events while assuring compliance with ODMHSAS standards and best practice requirements in managing case coordination. Ensuring protection of confidentiality as defined in 42 CFR Part 2 and HIPAA is a must. Consulting and cooperating with partners to facilitate linkage, referral, crisis management, and advocacy as well as follow up with the focus on attaining goals of both agency and residents. Measuring program outcomes by tracking program activities, monitor participant progress, managing data, and maintaining organized records. Preparing, assisting, and completing time sensitive reports. This includes but not limited to tracking resident enrollments, denials, discharges, and transfers. This position is also responsible for ODMHSAS required annual reporting. Meeting the needs of the local community by building, maintaining partnerships, and coordinating with local service providers to ensure residents are linked to supportive services. Assist in coordinating educational and/or community events for residents. Participating in ongoing effective communication and collaboration with residents while assisting with maintaining a healthy and safe living environment as well as housing stability.

Requirements

  • At least a Bachelor’s Degree in behavioral health, social work, sociology, public heath, or related field and three (3) years in behavioral health and/or social work including program development OR an equivalent combination of education and experience.
  • Proficient knowledge of Microsoft Office is a must.
  • NOTE: No substitution will be allowed for education below bachelor’s degree.
  • Must be willing and able to become Case Manager I (CMI) or Case Manager II (CMII) certified and earn housing plus endorsement.
  • Knowledge of and experience in working with behavioral health, substance abuse, child welfare systems, and community agencies and organizations
  • Strong time management and organizational skills
  • Ability to conduct several projects simultaneously
  • Excellent marketing, public relations, and program development skills
  • Superior oral and written communication skills
  • Ability to express ideas clearly, both orally and in writing
  • Ability to collaborate across systems to address operational and service issues
  • Skilled and experienced in coordinating and facilitating meetings
  • Exceptional interpersonal skills and ability to efficiently interact across systems
  • Ability to maintain accurate and detailed documentation
  • Ability to comply with Authority attendance policies
  • Possess Oklahoma Driver’s License with an acceptable driving record
  • Reliable transportation is a must
  • Near vision enough to read and write correspondence, read ledgers, and work with computerized reports, code books, plans, etc.
  • Hearing and speech sufficient to communicate clearly and effectively in person and by telephone
  • Physical range of motion and coordination for extensive work with files
  • Ability to access all types of houses, apartment complexes and buildings
