Now under new management, New Horizons offers competitive pay, amazing benefits, and comprehensive training. * Health Insurance (covers mental health) * Dental Insurance * Vision Insurance * Disability Insurance (STD; LTD) * Paid Time Off * Paid Holidays * Paid Sick Leave * Paid Training * Employee Assistance Program * Life Insurance * Retirement plan (403b) As a SOAR Specialist your responsibilities will include: * Responsible for completing interviews with clients to gather information to complete SSI/SSDI applications * Ensures all SOAR Medical Summary Reports for client applications are complete. * Accompanies clients to appointments at the Social Security Administration and maintains communication with Social Security Administration (SSA) and Disability Determination (DD) * Responsible for reviewing cases with Local SOAR Lead. Attend scheduled staffing with SOAR lead and SEFBHN * Complete SOAR medical summary report for individuals application utilizing internal/external medical reports, obtain current psychiatric Dr, APRN or NHTC medical director signature for summary report. * Monitors progress of benefit process and provides assistance as required * Conducts case management activities for assigned clients to include assessment clients to include assessment of strengths and needs, service planning, service brokering, service and care monitoring, and client advocacy * Communicates essential information regarding client care all members of the client's treatment team, both within and outside the NHTC continuum of care * Provide crisis intervention services as required Requirements New Horizons will need you to have the following qualifications: * Associates degree from an accredited college or university and 3 years' experience working with adults who have serious and persistent mental illness. * Bachelor's degree in a human service-related field - Four (4) years' experience may be considered in replacement of a degree * One (1) year of experience working with adults experiencing serious mental illness * Certified * Valid and current Florida Driver's License required with no more than 6 points in the past 5 years
