About The Position

The SNF Unit Manager coordinates and manages all nursing care activities in the Long Term Care facility, based on the philosophy of Arnot Health, using initiative and independent judgment to provide the best available services to residents. This position is responsible for directing and ensuring the health and well-being of residents by supervising and coordinating care delivery of unit staff. The manager coordinates care plans with physicians, nursing, and other healthcare professionals to ensure high-quality care in a respectful atmosphere. They also oversee and direct nursing documentation for the maximization of MDS components to ensure proper and accurate charting. This role supports the Director of Nursing (DON) in achieving unit goals and acts as the primary representative of the DON. Responsibilities include overseeing the nursing admission process, ensuring its completion, and completing the admission care plan within 24 hours. The manager is also responsible for the completion and maintenance of care plans and door plans, as well as floor coverage, oversight, coordination, and supervision in conjunction with the DON.

Requirements

  • Current New York State Registered Nurse Licensure.
  • Basic Life Support (BLS) required within 90 days of hire date.
  • Excellent communication and writing ability are required.
  • Excellent verbal and written communication skills.
  • Qualities of maturity, diplomacy, and ability to work well with others required.

  • Demonstrates strong clinical and supervisory skills, ability to work independently, highly motivated.
  • Management and organizational skills to include recognizing, analyzing and resolving problems.
  • Knowledge of legal aspects affecting nursing.
  • Ability to meet effectively with broad cross-section of individuals; knowledge of principles and methods involved in interpersonal relationships.
  • Trustworthy in safeguarding confidential information.
  • Total access to the medical record.
  • Knowledge of electronically medical record usage and MDS software.

Nice To Haves

  • BSN preferred.
  • Certification in nursing specialty desirable.
  • Minimum 2 years Long Term Care supervisory experience preferred.
  • Previous MDS 3.0 and PRI experience desirable.
  • Previous Long Term Care nursing administration management experience/knowledge is desirable.
  • Discharge planning experience recommended.

Responsibilities

  • Maintains confidentiality while serving as a professional liaison.
  • Represents the Medical Center in a professional, courteous manner to patients, families and members of the health care team while being sensitive to how others perceive both verbal and non-verbal communications.
  • Helps to develop, interpret and support Medical Center/Nursing department policies to members of the Long Term Care unit staff.
  • Maintains collaborative relationships with all members of the Long Term Care staff, attends weekly care conference meetings, and weekly Medicare A review with care team and therapies.
  • Demonstrates effective time management, flexibility and priority setting.
  • Promotes and ensures the delivery of quality resident care through competency based assignments and supervision of staff.
  • Supports the philosophy and objectives of the Long Term Care Department.
  • Ensures the utilization of the nursing process in maintaining quality of standards of practice and standards of care.
  • Develops leadership skills of staff and delegates responsibility appropriately.
  • Demonstrates appropriate problem solving and decision making skills.
  • Assists in responding to staff regarding grievance and problems and refers them to appropriate resources as necessary.
  • Promotes a healthy working environment through effective communication, conflict resolution, and staff empowerment.
  • Promotes inter-shift communication among staff members to achieve continuity of nursing care.
  • Serves as a liaison between Long Term Care staff, nursing units and ancillary departments in conjunction with DON to maintain a collaborative working relationship.
  • Collaborates with DON in organizing and conducting unit staff meetings.
  • Collaborates with the DON in developing annual written goals and objectives for the Long Term Care department.
  • Collaborates with the DON with maintaining the nursing quality improvement process to improve resident care to the greatest extent possible using an approach, which is systematic, planned, and outcome oriented.
  • Collaborates with DON with selection, development, supervision, evaluation, and counseling of employees as appropriate.
  • Coordinates scheduled staffing pattern to provide continuity of resident care.
  • Assists the DON with implementing appropriate review systems for competency-based appraisals of staff.
  • Participates in Nursing/Medical Center and unit committees as appropriate.
  • Will be responsible for the completion of all RN/LPN and CNA annual evaluations as assigned.
  • Will be responsible for RN/LPN and C.N.A. disciplinary actions, as needed.
  • Reports to QI team for monitoring patient care audits as assigned.
  • Assists in the orientation of new employees, and serves as a resource person as needed.
  • Assigns, assures attendance, and encourages participation at continuing education/staff development programs.
  • Responds and assumes appropriate role in emergency codes/situations.
  • Follows resident through discharge home, where appropriate.
  • Works with admission coordinator to Coordinate admissions of short and long term stay residents, and facilitate discharge of these same patients.
  • Attends resident review meetings for the restorative rehabilitation and designated short term stay residents, and care planning meetings for the long term care unit as appropriate/required.
  • Assists clinical staff with documentation for MDS based Case Mix to ensure appropriate classification of residents for reimbursement.
  • Handles financial, personnel and medical records or other facility information in a confidential manner.
  • Attends staff meetings and required or preferred in-service training.
  • Generates and develops ideas, which improve the quality of care for residents or increase position productivity and satisfaction. Encourages involvement and recognizes staff for making exceptional contributions.
  • Performs all position duties according to safety rules and practices required for this position, and as required to ensure the general safety of staff, residents and visitors of this facility.
  • Performs assigned duties according to the service excellence standards and goals established for this position, and for all staff members of this facility.
  • Exhibits the highest level of customer service towards staff, residents, and visitors and assure that all staff do the same.
  • Performs other position duties as requested be supervisor, management staff or Administrator.
  • Demonstrates awareness of need for self-growth by assessing strengths and weaknesses and establishing written goals and objectives.
  • Serves as a professional role model for staff and students by maintaining knowledge and expertise to clinical area.
  • Observes Medical Center dress code: Wears identification badge while on duty and maintains a professional appearance at all times.

  • Is familiar with and demonstrates Quality Improvement principles and participates in the process as appropriate.
