CHCA is seeking a full-time Practice Office Care Coordinator to join our Skilled Nursing Facility Practice team at our Fair Oaks office. As Care Coordinator, you will provide vital administrative support for our operations within Skilled Nursing Facilities (SNFs). You will coordinate day to day operations and ensure good business practices while healthcare professionals focus on providing healthcare. This is a fast-paced environment with a strong commitment to excellence, where attention to detail, adaptability, and a proactive attitude are essential for success. The regular work schedule will be Monday - Friday.

Requirements

  • 5-7 years of related medical office experience, working with MDs/NPs
  • Skilled nursing facility or AL facility experience
  • College degree preferred, high school or equivalent required
  • Medical Assistant or Medical Office Assistant certificate preferred
  • Familiarity with medical and dental terminology and EHR systems (eCW preferred)
  • Knowledge of HIPAA regulations
  • Respect of cultural and religious practices of patients

Responsibilities

  • Support the implementation of organizational policies and procedures within the SNFs to ensure consistent patient care delivery
  • Collaborate with providers and program leadership to address workflow issues and improve patient care processes with guidance from the Regional Director and Medical Director
  • Assist in coordinating provider SNF schedules and patient care coverage to ensure adequate access to care and efficient operations
  • Participate in the recruitment process for providers by assisting with screening and coordinating interviews with leadership.
  • Monitor provider charting and documentation practices to ensure compliance with documentation standards and support accurate patient care records.
  • Support provider performance by assisting with productivity monitoring and related incentive or bonus programs.
  • Provide day-to-day oversight and support to staff to ensure efficient patient flow and high-quality patient care.
  • Participate in Quality Assurance (QA) and Quality Management (QM) activities related to care, patient safety, and service quality.
  • Work with healthcare providers and staff to identify and address workflow challenges affecting patient care delivery.
  • Serve as a point of contact for by responding to questions related to patient services and processes.
  • Ensure timely completion of provider documentation and compliance tasks in accordance with applicable regulatory standards, including Title 22 requirements.
  • Provider Appointment Coordination: Coordinate appointments for specialty providers (dental, podiatry, mental health, etc) with facilities. Ensure all patients are scheduled and seen within Title 22 Regulations. Ensure all patient services and billing information is collected and submitted timely and accurately.
  • Administrative Support: Efficiently manage data entry, prepare reports, and process invoices.
  • Communication: Professionally handle incoming calls, respond to information requests, and liaise with clients as required.
  • Records Management: Verify, file, and maintain medical records electronically, ensuring accuracy and confidentiality.
