About The Position

Duly’s Value-Based Care organization is focused on delivering high-quality, coordinated care across the post-acute continuum. Reporting to the Director, Post-Acute & Advanced Care (or designee), the Supervisor, SNF & Geriatrics FFS is responsible for supporting the operational success of the Geriatrics fee-for-service (FFS) program by ensuring consistent SNF coverage, strong coordination with Post-Acute teams, and reliable administrative and operational support for Geriatricians. This role partners closely with both the Post-Acute team, SNF RN Case Managers, and Geriatric physician leadership to ensure Geriatricians are appropriately deployed, supported, and enabled to focus on patient care. The Supervisor serves as a key operational bridge between physicians, SNFs, and care management teams.

Requirements

  • BA/BS degree in Healthcare Administration, Business Administration, or a closely related discipline required OR an equivalent combination of post-secondary education or job-related experience may substitute on a year-for-year basis
  • A minimum of 3 years of related work experience in a medical clinic, hospital setting, or similar environment required, of which 1 year supervisory/managerial experience in a medical clinic, hospital setting, or similarly regulated environment preferred
  • Proven track record of delivering operational results in a matrixed environment
  • Experience working directly with physicians and clinical staff
  • Proven experience in executing and implementing initiatives in a healthcare setting
  • Strong project management skills with a demonstrated ability to drive projects to successful completion
  • Exceptionally detail-oriented with a high degree of objectivity and analytical skills to ensure accuracy of reports and data.
  • Excellent communication and interpersonal skills with the ability to influence and inspire others
  • Knowledge of value-based care and fee-for-service principles, healthcare operations, and regulatory requirements
  • Project management expertise and ability to foster collaboration across diverse teams
  • Exceptional written, verbal and interpersonal communication skills
  • Ability to effectively communicate with employees, patients and other individuals in a professional and courteous manner
  • Proficient in Microsoft Office Suite products including Excel, Word, PowerPoint and Outlook, plus a variety of other word-processing, spreadsheet, database, e-mail and presentation software.

Responsibilities

  • Partner with the Post-Acute team and SNF RN Case Managers to ensure appropriate Geriatrician coverage across skilled nursing facilities.
  • Maintain and regularly update Geriatrician coverage maps by SNF, including primary and backup coverage.
  • Support day-to-day coverage adjustments related to PTO, illness, schedule changes, or census fluctuations.
  • Serve as a point of contact for SNF partners regarding Geriatrics coverage coordination and operational questions.
  • Provide administrative and operational support to the Geriatrics FFS physician team, including:
  • PTO tracking and coordination
  • Schedule support and coverage planning
  • Distribution of daily SNF census reports
  • Coordination of basic operational needs and requests
  • Ensure Geriatricians receive timely, accurate information needed to perform their roles effectively.
  • Help establish and reinforce clear expectations related to schedules, coverage responsibilities, and communication standards.
  • Work closely with the Post-Acute leadership team to align Geriatrics operations with broader SNF and post-acute strategies.
  • Support Post-Acute initiatives by providing data, coordination, and operational follow-through as needed.
  • Assist with issue escalation and resolution related to SNF operations, physician coverage, or workflow gaps.
  • Participate in recurring meetings with Post-Acute leaders and SNF partners as appropriate.
  • Distribute and maintain recurring operational reports (e.g., daily census, coverage summaries).
  • Ensure consistent communication between Geriatricians, SNF Case Managers, and Post-Acute leadership.
  • Identify opportunities to improve workflows, reduce manual work, and standardize processes related to Geriatrics and SNF operations.
  • Support documentation of processes and contribute to playbooks or SOPs as the program matures.
  • Provide day-to-day guidance and oversight to administrative or coordinator staff supporting the Geriatrics program.
  • Support onboarding and training of new team members as needed.
  • Reinforce accountability, professionalism, and a service-oriented mindset across the team.
  • Works with Geriatricians to assist with scheduling meetings with patients and/or writing letters to/for patients
  • Reviews reports to ensure target metrics are achieved and processes are being followed
  • Conducts trainings and refreshers regarding regulatory compliance, ensures annual OSHA trainings occur and employees have all required certifications current in their files
  • Monitors transportation and housekeeping activities
  • Collaborates with the Leadership Team and Administrators in relation to strategic business planning
