Position Highlights • Monday–Friday | 8:00 AM–5:00 PM • Shared call rotation • In-person and virtual visits supported by Medical Assistant • Manage complex patients with ongoing follow-up throughout stay • Collaborate with SNF team to facilitate care coordination • Implement discharge planning to improve patient outcomes • Signing bonus, CME, and relocation available. Requirements
