Provides comprehensive medical services for patients in a program of home-based and clinic care designed to serve the needs of medically fragile patients with complex conditions. Patients (and their family members) will need well-coordinated medical care that addresses their personal goals for well-being and quality of life. Care will include primary care, urgent care and Hospital at Home care, as well as collaboration with nurse case managers, care coordinators, behavioral health specialists and consulting physicians and other healthcare professionals who comprise the patients care team. The Complete Care Medical Director and Clinic Lead physician lead the care teams in providing efficient and compassionate care.
