About The Position
The RWJBH Social Impact and Community Investment (SICI) SNAP Navigator will play a crucial role in assisting patients and community members with screening, providing eligibility information, and facilitating enrollment in the Supplemental Nutrition Assistance Program (SNAP) benefits. This position is designed to operate within RWJBH clinical spaces, outreach events, and in collaboration with community partner organizations across the assigned counties. The Navigator will work in alignment with RWJBH's commitment to combat food insecurity, ensuring that healthy food options are accessible to those in need within the community. The ideal candidate for this role will be an enthusiastic team member who is passionate about making a positive impact on the lives of individuals facing food insecurity. The Navigator will be responsible for receiving referrals from both clinical and non-clinical staff, as well as community partners, to screen and enroll individuals in SNAP benefits. This includes assisting with the completion of new and renewal benefits applications on behalf of patients and community members, providing follow-up assistance regarding the status of applications, and coordinating scheduling with community partner organizations to deliver services effectively. Additionally, the SNAP Navigator will maintain and foster relationships with partner organizations, participate in ongoing training to stay updated on program eligibility requirements and application procedures, and collect enrollment data for departmental and state reporting. This role is essential in ensuring that the community has access to the resources they need to combat food insecurity and improve overall health outcomes.
