About The Position

Personal Health Care is seeking a Licensed Practical Nurse (LPN) or Registered Nurse (RN) to provide one-on-one skilled nursing care to pediatric clients in home and school settings in the Philadelphia area. This full-time position offers flexible scheduling and the opportunity to work up to 40 hours per week, with a focus on delivering compassionate and high-quality care.

Requirements

• Current Pennsylvania Skilled Nursing License
• Current CPR Certification
• Ability to lift at least 50 pounds
• Pass State and Federal Criminal Background Checks
• Up-to-date Physical and Two-Step PPD or Chest X-ray
• Pass Skilled Nursing pre-employment test
• Form of Transportation to Client's Residence

Responsibilities

• Assess patient's need for safety measures and appropriate interventions.
• Implement the Nursing Care Plan as outlined by the Primary Nurse.
• Implement medication changes according to verbal and/or written physician orders.
• Administer prescribed medications and treatments.
• Accurately observe and promptly report changes in the patient's physical or mental status to the Nursing supervisor.
• Accurately record nursing observations, activities, and treatments on the appropriate forms.
• Follow a physician's plan of care under the supervision of the Director of Nursing.
• Communicate with family members, in-office staff, and all supporting health professionals.
