About The Position

Reporting to the on-duty Charge Nurse, the LVN administers nursing care to patients in an efficient, safe, and courteous manner using proper nursing procedures. This job description is intended to identify some of the primary duties and responsibilities. Mayers Memorial Hospital District (MMHD) reserves the right to modify, supplement, delete or augment the duties and responsibilities specified in this position description, at MAYERS MEMORIAL HOSPITAL DISTRICT’S sole and absolute discretion.

Requirements

  • High school diploma or equivalent.
  • A current California nursing licensure that is in good standing.
  • Maintains current BLS certification.
  • Professional behavior is demonstrated, both when on duty and as a representative of MMHD outside of normal work hours.
  • Ability to work with others, at all levels within the organization, and collaborate effectively.
  • Above-average interpersonal, problem-solving, and written and oral communication skills.
  • A positive working relationship with patients, visitors, and facility staff is demonstrated.
  • Organizational ability and time management is demonstrated.
  • Produces deliverable products on time, within budget with minimal direction.
  • Demonstrates the ability to compile and organize data using Microsoft Office Applications. Ability to make appropriate recommendations or conclusions, given the data obtained.
  • Maintains an organized set of records, upon request, for the functions performed within this job's scope.
  • Communicates appropriately and clearly to internal and external customers.
  • Performs all assigned tasks accurately, including proper spelling and grammar, and reference materials.
  • Takes direction from others to include those employees of other departments with which he/she works on a temporary basis in accordance with the chain of command.
  • Maintains all information as confidential as appropriate, including payroll, personnel information, and other confidential information obtained while performing job duties.
  • Demonstrates ability to effectively use office machines in the performance of job functions.
  • Using telephone etiquette, answers phone calls and assists the public in a professional and friendly manner.
  • A willingness to work beyond normal working hours and in other positions temporarily when necessary.
  • Must function independently, have personal integrity, have flexibility and the ability to work effectively with other personnel, residents, and support agencies.
  • Sits, stands, bends, lifts, walks, and moves intermittently during working hours.
  • Able to lift 50 lbs.

Responsibilities

  • Conducts an individualized patient assessment, prioritizing the data collection based on the patient’s immediate condition or needs within time specified by facilities policy and procedures or protocols. Notifies Charge Nurse and Provider of abnormal findings.
  • Develop and update an individualized resident plan of care that reflects collaboration with the resident, family, and other members of the healthcare team.
  • Performs appropriate treatments as ordered by providers accurately and timely.
  • Performs all assigned tasks accurately. Keep the work area and assigned rooms clean.
  • Provides individualized resident/family education.
  • Documents patient assessment findings, physical/psychosocial responses to nursing intervention and progress towards problem resolution.
  • Initiates emergency resuscitative measures according to appropriate resuscitation protocols.
  • Maintains confidentiality in matters related to residents, family, and facility staff.
  • Provides care in a non-judgmental, non-discriminatory manner that is sensitive to the patient’s and family’s diversity, preserving their autonomy, dignity, and rights.
  • Reports patient condition to appropriate personnel during each shift.
  • Adheres to standards of safety and infection control in delivery of patient care.
  • Assists nursing staff in other departments, as requested and if qualified, when need arises.
  • Maintains current CPR certification.
  • Works with other departments in coordinating smooth transfer of patients for procedures or diagnostic tests.
  • Assumes professional responsibility for assuring patient care is of the highest standards possible. Communicates questions about appropriateness of medical or nursing care through chain of command, as needed.
  • Follows procedure for medication administration and narcotics control. Documents, medications, and nursing care in a timely and comprehensive manner.
  • Assists with direct patient care, as needed. Assures that call lights are answered promptly and that nursing care assigned to CNA’s is completed and documented according to facility policy & procedures.
  • Responds to emergency situations appropriately; notifies all staff, family & providers in a timely manner. Carries out emergency interventions, orders, and evaluations quickly and efficiently. Documents resident events.
  • Works under the direction of the Charge Nurse to provide nursing care to Skilled Nursing residents. End of shift walking rounds to maintain continuity of care.
  • Adheres to dress code; appearance is neat and professional.
  • Completes annual education requirements if applicable.
  • Maintains regulatory requirements.
  • Wears identification while on duty.
  • Attends annual evaluation and participates actively in this process.
  • Reports to work on time and as scheduled; completes work in designated time.
  • Attends meetings as appropriate.
  • Exhibits the mission, ethics, and goals of Mayers Memorial Hospital District in the performance of job duties.
