Requirements

  • Active Colorado Registered Nurse (RN) license (required)
  • Strong clinical judgment and comfort working independently
  • Solid time-management, communication, and documentation skills
  • Reliable vehicle, valid driver's license, and auto insurance
  • A commitment to ethical, patient-centered nursing care

Nice To Haves

  • Home health or skilled nursing experience preferred (training available)
  • Familiarity with Medicare-skilled services preferred

Responsibilities

  • Provide skilled nursing services in patients' homes
  • Complete Start of Care, recertification, and routine skilled visits
  • Perform comprehensive assessments, medication management, wound care, and patient education
  • Coordinate care with physicians, therapists, caregivers, and families
  • Document accurately and efficiently using Homecare Homebase (HCHB) EMR
  • Participate in interdisciplinary care planning and collaboration
  • Travel locally within Mesa County (mileage and drive time reimbursed)
