We know home health can feel intimidating if you’ve never done it before — you will not be sent out alone unprepared. Requirements

  • Active, unrestricted RN license
  • At least 1 year of nursing experience in any clinical setting (hospital, SNF, clinic, rehab, etc.)
  • Willingness to learn and work independently
  • Reliable transportation, valid driver’s license, and auto insurance
  • Ability to travel within the branch coverage area
  • ability to lift and pull 50-100 lbs

Nice To Haves

  • Home Health or Hospice experience
  • Experience with HomeCare HomeBase (HCHB)
  • ASN or BSN

Responsibilities

  • Perform start-of-care visits and routine nursing visits
  • Complete patient assessments and develop individualized care plans
  • Provide education to patients and caregivers on diagnoses, medications, and treatments
  • Monitor progress and adjust care plans as needed
  • Coordinate care with physicians, therapists, and interdisciplinary team members
  • Document visits and clinical findings in the electronic medical record
  • Identify changes in condition and communicate appropriately
  • Ensure compliance with HIPAA and safety standards
