Requirements

Active RN license with 1+ years of clinical experience
Comfort traveling within the territory (about 50% travel)
Reliable transportation, valid driver’s license, and auto insurance

Nice To Haves

Home Health or Hospice experience is a plus
Associate or Bachelor’s in Nursing preferred
Experience with HomeCare HomeBase is a bonus

Responsibilities

Lead your caseload with autonomy—assess, plan, and deliver care tailored to each patient
Provide compassionate guidance and support to patients and families
Coordinate care with doctors, therapists, and caregivers to keep everyone on the same page
Adapt plans as patients’ needs evolve, ensuring the best outcomes
Maintain HIPAA-compliant documentation while being the trusted voice patients rely on
