Terros Health is hiring an Arizona Independent Licensed Clinician for our Clinical Site Manager position in Phoenix, AZ. Location: 4909 E McDowell Rd, Phoenix, AZ 85008 Schedule: Full-Time, 40 hours/week Mon - Fri, 9:00am - 5:00pm General Mental Health / Substance Use Disorder Experience Preferred Experience Treating Co-Occurring Disorders Preferred Behavioral Health Experience Required 3 Years of Supervisory Experience Preferred Arizona Licensed: LCSW or LPC (Independent License Required) The Site Manager must possess a master’s degree in the mental health or related field as well as state licensure. Employment is contingent upon the acquisition of licensure within 90 days of hire. The Site Manager must have at least 3 years’ experience working with populations diagnosed with co-occurring disorders. The Site Manager offers administrative and clinical oversite of one of the Terros Health’s LADDER (Life Affirming Dual Diagnosis Education and Recovery) programs ensuring the safety of patients and staff as well as compliance with all state licensure requirements. The Site Manager is responsible for overseeing the daily operations, staff schedule, client engagement and clinical oversight of the program This position reports to the Director of Recovery Services.

Requirements

  • Master’s degree, i.e., MA, MC, MSW, in a behavioral health field, Arizona license or eligible for Arizona license. license
  • At least three (3) years of experience working with persons diagnosed with co-occurring disorders.
  • Experience in group therapy and has the ability to facilitate group therapy which is an essential component of addiction treatment therapy.
  • Strong oral, written and interpersonal communication skills.
  • Ability to assist patients, convey information, resolve issues, cultivate connections, build solid relationships, and express empathy throughout the patient experience.
  • Must have or be able to attain CPR/First Aid certification within 60 days of assuming role.
  • Proficiency using computer-based systems including electronic health records, Microsoft Outlook, and other Microsoft Office products.
  • Must have valid Arizona driver’s license, be 22 years of age with minimum 3 years driving experience, and meet requirements of Terros Health’s driving policy.
  • Must have a valid Arizona Fingerprint Clearance card or apply for an Arizona fingerprint clearance card (Level 1) within 7 working days of assuming role.
  • Must pass a TB Test.

Responsibilities

  • Responsible for overseeing all daily operations including maintenance of the staff schedule to ensure daily coverage, structure, and organization of the program
  • Required to attend team meetings, Terros Health Leadership Team (THLT) and all other meetings as assigned.
  • Required to cooperate will key individual in the organization.
  • Responsible for establishing and maintaining a good working relationship with referral sources to ensure referrals continue on an ongoing basis.
  • Ensure patients begin the program within 3-5 working days of receipt of a referral; or patient’s preference of start date.
  • Responsible for maintaining the minimum daily census (20) requirement consistent with the annual budget through appropriate outreach and coordination of care for patients who are not attending on a regular basis.
  • Adheres to the organization policies and procedures.
  • Responsible for maintaining compliance with licensure requirements as well as state, federal, Commission on Accreditation of Rehabilitation Facilities (CARF)- and contract-specific regulations.
  • Responsible to ensure all patient documentation is completed by direct reports within established guidelines including but not limited to the following: progress notes (same day of service) and accompanying billing information. All documentation will be completed by end of shift. Corrections to be completed within 24 hours of notice.
  • Responsible for oversight of treatment planning staffing’s at least 1 time per month per patient and adjusting Level of Care ( LOC) as clinically indicated.
  • Required to attend patient staffing’s as needed and assist direct reports in coordinating interventions and oversee implementation of those recommended interventions.
  • Conducts monthly quality management and data validation audits to ensure all direct reports documentation is timely and adhering to the state, federal, CARF and all contract specific regulations.
