The CCS Site Coordinator is responsible for all tiers of the Connected Community Schools (CCS) Model -Universal, Targeted and Individual supports. The CCS Site Coordinator will serve as a liaison between the district staff and CCS project management, collaborate with the school district and facilitate an assessment process that aligns with the districts strategic plan and with local and regional community resources to reduce barriers to student well-being, achievement, and school connectedness. The CCS Site Coordinator will organize and facilitate district/school level LINK and CCS meetings, and facilitate school and classroom programming and family engagement activities throughout the school district. This position also serves as the primary point of contact for the LINK operating system, responding to all incoming LINK referrals, completing individualized needs assessments, and making appropriate referrals to resources and community services.
