About The Position

The Site Coordinator for the Community Living Program at Catholic Community Services of Southern Arizona is responsible for facilitating the implementation of Individual Service Plans (ISP) and overseeing various administrative duties. Requirements

  • High School diploma or equivalent.
  • One year of directly-related work experience.
  • Excellent verbal and written communication skills.
  • Strong organizational abilities for staffing and scheduling.
  • Knowledge of DES/DDD Residential Program Monitoring rules.
  • Experience with writing and implementing objectives and skill plans per ISP guidelines.
  • Ability to lift 60 lbs 15% of the time.
  • Valid driver license and proof of insurance.
  • Ability to obtain and maintain Arizona Level One Fingerprint Clearance Card and FBI National Criminal Records History Report.

Responsibilities

  • Facilitates the implementation of Individual Service Plans (ISP).
  • Participates in administrative duties including staff training, scheduling, and oversight.
  • Coaches and works with a team of Direct Support Professionals.
  • Provides support to individuals with daily activities, medication administration, and personal care.
  • Accompanies individuals on community outings and provides transportation as needed.
  • Completes staff performance evaluations and provides constructive feedback.
  • Ensures compliance with Department of Economic Security (DES) and Division of Developmental Disabilities (DDD) regulations.
  • Maintains client records to meet DES/DDD guidelines and agency policies.
  • Prepares and completes resident ISPs and skill training objectives.
  • Writes and implements Behavior Treatment Plans (BTP) for residents.
  • Conducts regular staff meetings to provide information and feedback.
  • Ensures safe and appropriate program operations and reports issues to management.
  • Maintains accurate attendance and reporting at ISP meetings.
  • Works with the Program Manager to ensure adequate data collection using the Therap system.
  • Ensures client medical needs are met and documented accurately.
  • Completes monthly documentation for DDD Support Coordinator and other team members.
  • Maintains financial accountability for program household and client accounts.
  • Participates as an active member of individual ISP teams and maintains professional contacts.
  • Provides direct care as needed to meet program requirements.
  • Shares on-call duty with Lead DSPs for emergencies.
  • Communicates consistently with staff regarding client information and documentation requirements.
  • Responds to employee issues and completes performance evaluations under Program Manager guidance.
  • Promotes agency policies and procedures within the community.
  • Ensures quality indicators are present in the program.
  • Communicates regularly with CLP Administration on staffing and safety issues.
  • Completes weekly inspections of apartments or homes and van inspection logs.
  • Creates a monthly activity calendar with residents and staff.
