About The Position

To coordinate the care and treatment of singe ventricle (SV) heart defect patients in both the inpatient and outpatient setting. The SV coordinator is responsible for following the SV patients in both the ambulatory and inpatient settings, from admission to discharge and subsequent follow-up visits; maintaining contact with patient and family throughout various touchpoints. The SV coordinator will be responsible for the CHAMPS program, including, but not limited to, assessment, enrollment, monitoring and evaluation of the program. The coordinator will coordinate SV clinic visits and testing as well as run the SV clinics as scheduled. The SV coordinator meets with and educates parents in the NICU and will maintain communication with parents after discharge. Under the direction of a board certified cardiologist and heart center leadership, the SV coordinator collaborates with the multi-disciplinary team to provide patient care and education to staff and families. The coordinator will collaborate with the West Texas coordinator to ensure continuity of care for patients in the West Texas area. The SV coordinator acts as a liaison between the multi-disciplinary team and the cardiologist. Also works with the Heart Center leadership for program growth and development related to the care of patients with single ventricles heart defects.

Requirements

  • Associate of Nursing degree required. BSN preferred.
  • Minimum 2 years of experience in pediatric cardiology.
  • Demonstrated knowledge of congenital heart disease.
  • Demonstrated skill in patient care, communication, and interpersonal relationships.
  • Demonstrated ability in patient education.
  • Physical and mental skills to carry out job responsibilities.
  • Current State License R.N.
  • BLS required, PALS required

Nice To Haves

  • BSN preferred.
  • Cardiac ICU experience preferred.
