Requirements

  • Bachelor's degree in social work, psychology, or related field with one year of case management experience required; high school diploma with five years of related experience may substitute; Master's degree preferred.
  • Experience with program-specific populations preferred.
  • Experience working with low-income and homeless individuals.
  • Availability to work flexible days and hours.
  • Ability to work independently and respond quickly in times of crisis.
  • Ability to relate well to individuals with disabilities and older individuals.
  • Ability to handle complex cases and problem solve.
  • Ability to work effectively with diverse ethnic, racial, and social backgrounds.
  • Proficiency in Microsoft Office applications.
  • Strong verbal and written communication skills; valid driver's license, reliable transportation, and proof of liability insurance.

Responsibilities

  • Operate as a member of the program team.
  • Conduct comprehensive needs assessments and coordinate services focused on obtaining permanent housing for all clients.
  • Conduct all case management with a Harm Reduction, Client Centered philosophy.
  • Monitor ongoing client services and advocate as needed.
  • Provide case management services in office or field-based settings.
  • Handle complex cases and conduct follow-up as required.
  • Maintain confidential case records and complete required documentation.
  • Maintain cooperative relationships with service providers and community organizations.
  • Other duties as assigned within the guidelines of this position.
