Responsibilities

Developrelationshipsandcollaboratewiththeprimarycareteamstoconductsocialservicesassessmentstodeterminetheappropriateneedsforeachmember
Plan, coordinate, manage and implement support packages to help members deal with socioeconomic and medical barriers
Navigatemanagedcareplansforcommunityservicesandprograms
Casemanagementforsocialandbehavioralcaretoallowmemberstoself-managehealthandsocialservicesupport
