Responsibilities

  • Developrelationshipsandcollaboratewiththeprimarycareteamstoconductsocialservicesassessmentstodeterminetheappropriateneedsforeachmember
  • Plan, coordinate, manage and implement support packages to help members deal with socioeconomic and medical barriers
  • Navigatemanagedcareplansforcommunityservicesandprograms
  • Casemanagementforsocialandbehavioralcaretoallowmemberstoself-managehealthandsocialservicesupport
  • Accurateandtimelydocumentationofpatientencountersandsessionsinallclinicalmanagementsystems
  • Proactivelyidentify methods toimprove ArchWellHealth's approachbased onfeedbackandregularlyconductedsurveys
  • Support advance care transitions with members and their families
  • Assist members with access to state-based prescription programs and other benefits
