Hands of Heartland is looking for compassionate and caring individuals to share their home with a person with developmental disabilities as a Shared Living Provider. We are looking for someone in South Sioux City, NE area.

Requirements

• Must be 19 years old.
• Must have a valid driver’s license and car insurance.
• Must be able to pass all background checks.

Nice To Haves

• Prefer at least one year experience working with individuals with developmental disabilities.

Responsibilities

• Provide supports to a person with a developmental disability in your home to ensure their wants and needs are met in compliance with the Nebraska Medicaid waiver program.
• Assisting with activities of daily living
• Meal planning and preparation
• Providing transportation
• Assisting with medications
• Implementing programs in accordance with the persons Individual Support Plan.
