About The Position

The Service Navigator / Case Manager at Corazon Ministries is responsible for assisting clients in navigating the services offered at the Day Center, ensuring they receive the necessary support for their basic needs and case management. This role involves collaboration with various outreach and case management teams to provide a continuity of care for individuals facing challenges such as mental health issues and homelessness. The position requires a commitment to the organization's mission of providing unconditional love and justice through various support services.

Requirements

  • High school diploma or equivalent.
  • Valid driver's license and insured vehicle.
  • 2+ years in a customer service focused environment.
  • Experience with homelessness or low-income populations is a strong plus.
  • Positive attitude and self-motivated.
  • Dependable and punctual team player.
  • Good organizational skills and attention to detail.
  • Competent user of MS Office applications, email, and social media platforms.
  • Ability to work independently and make appropriate decisions.
  • Mental resolve to deal with complex, highly emotional, and/or stressful situations.

Responsibilities

  • Assist clients and address their basic needs.
  • Collaborate with harm reduction outreach manager and case managers to assist with placements from the field.
  • Print directions, IDs, and miscellaneous requests as needed.
  • Triage clients to identify their needs and barriers to accessing those needs.
  • Maintain the restroom sign-in and check bathrooms regularly unless a volunteer has been assigned this specific task.
  • Assist with serving lunch or other campus needs, including organizing supplies and maintaining cleanliness.
  • Assist the director with needed tasks.
  • Input data daily, weekly, or monthly into HMIS to track client interactions and services provided.
  • Participate in organization events and represent the organization with neighborhood partners.
  • Assist with organizing, labeling, and making inventory of donations.
  • Identify necessary supplies for the Day Center and communicate those weekly with the director.
  • Assist with locating the best price/location of needed supplies and place orders when required and approved by leadership.
  • Ensure all rooms in the Day Center are organized, clean, and presentable at the end of each day.
  • Complete ID recovery needs and processes with clients as needed.
  • Provide case management to clients and assist them with referrals to shelters, treatment centers, detox, etc.
  • Collaborate with community partners to ensure the best placement for clients being case managed.
