About The Position

The QIDP provides service facilitation for assigned individuals in Medicaid waiver programs (Developmental Training, CILA, and Home-Based Support). This includes developing support strategies to support identified outcomes in the person-centered planning process and coordinating comprehensive services based on outcomes identified as important to/for each individual. The QIDP works cooperatively with DT management and reports directly to the Director of Residential Services and/or Director of Developmental Training. Essential Functions : Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions. Service Facilitator - QIDP Follow DHS Rule 119, HBS regulations, and CARF guidelines in providing service facilitation. Facilitate the development of Implementation Support Strategies to meet outcomes for designated individuals in the Person-Centered Process Support and encourage independence and self-reliance, allowing program participants to achieve their greatest potential by giving them opportunities to make informed decisions and to make choices about their daily routines Coordinate and communicate with individuals receiving services, residential providers, participant’s guardian/family/ caregiver and other agency staff to assure efficient provision of services Coordinate and communicate with other community service agencies to assure that individuals are receiving comprehensive support services Coordinate and communicate regularly with DT Managers, CILA Home Managers, Coordinators and other DSPs for input on issues related to the implementation of the Person-Centered Plan Assure that annual assessments are completed in a timely manner (i.e. ICAP, Risk assessments) Provide HBS service facilitation when indicated by HBS funded individuals/ families Complete billing and monitor supports for individuals according to HBS regulations Transport individuals as needed, following Transportation Policies Assure that individual support strategies and outcomes are addressed during routine daily activities and monitor related progress. Communicate and meet regularly with the DT team to address issues related to individual outcomes and supports concerns, and the overall functioning of the program Complete incident /injury reports, critical incident and seizure reports in a timely manner Contribute to and assure development of a person-centered culture Ensure safety of program participants Be alert to individual medical needs, provide first aid and contact supervisor, or executive staff member and or agency health professional when necessary Assist with the orientation of new participants Assist in the training of DSPs and ongoing training of staff members in DT and CILA Other Responsibilities Follow DSP training and job description Provide coverage at DT Sites or CILA’s as needed Responsible for attending trainings to obtain QIDP status Acquire CEU’s annually, document CEU’s to maintain QIDP status Responsible for attending trainings to meet annual requirements: First Aid, CPR, CPI, Safety Respect persons served and promote a positive image of person with disabilities Perform other duties as assigned

Requirements

  • Bachelor’s Degree in Special Education or a human services related field
  • Minimum of 1 year experience working with individuals with ID/DD
  • Valid Illinois Driver’s License
  • Maintain minimum insurance requirement
  • Pass physical function test
  • Must have or be able to obtain CPR, Heimlich Maneuver, First aid qualifications
  • Data entry skills using programs such as Word, Paychex, and Paychex Flex
  • Solid communication and interpersonal skills

Responsibilities
