About The Position

Our next Service Facilitator- Comprehensive Community Services  will be responsible for the coordination of services for youth in the Comprehensive Community services program of Washington County and to ensure the needs of youth and family are met by providers to facilitate greater overall mental health and well-being.

Requirements

  • Bachelor’s degree in social work or related field required, master’s degree in social work or related field preferred.
  • Licensure as a CSW, APSW, LPC-IT, LMFT-IT, LCSW, LPC, or LMFT preferred.
  • Minimum of two (2) years of post-bachelor’s experience working with children, adolescents and families preferred.
  • Proficient in the usage of computer software; demonstrate an understanding of computer file systems and computer software packages, e.g., Microsoft Office, Excel, Outlook, Teams, ADP, etc.
  • Must be honest, dependable, and able to meet deadlines; Self-motivated and able to work independently.
  • Physical exam, drug screen, motor vehicle report, and background checks are required for this position.
  • A valid Wisconsin Driver’s License or occupational driver’s license, reliable transportation and insurance is required. Required reliable transportation to conduct on-site learning throughout the state.

Responsibilities

  • Performs clinical assessments, including intake interviews and screens for new referrals using trauma informed and recovery-oriented perspective.
  • Develops, directs, and implements client treatment plans, interventions, skill training and crisis management.
  • Links clients and families with community resources and support systems, including referral to other services and monitoring of involvement.
  • Provides individual and family psychoeducation and skill building by maintaining face to face contact with youth and family in the community and home settings.
  • Sets appropriate expectations for family involvement upon admission.
  • Supports families in crisis resolution and de-escalation through developing appropriate crisis plan.
  • Establishes and maintains working knowledge of the WCHSD programs, resources, and supports.
  • Helps the individual and family negotiate access to services and resources in the community and minimize barriers.
  • Facilitates bringing together service teams for regular and consistent collaboration to best meet the needs of the client and family.
  • Completes all necessary paperwork timely and utilizes trauma informed, strength-based language.
  • Maintains client files in accordance with program policies and participates in quarterly review of files.
  • Monitors collection of outcome data to measure program performance. Obtains accurate and complete data.
  • Supports program coordinator in compiling data for purposes of program development.
  • Actively participates in clinical consultation individually and team driven.
  • Displays organization and time management skills to meet all deadlines.

  • Participates in assigned meetings, events and learning as required.
  • Other duties as assigned, including serving in a coverage role for other department members.
