About The Position

The Service Coordinator is knowledgeable about the Early Steps Program and serving infants and toddlers from birth to age three with special needs. Maintains a caseload, acts as the primary Service Coordinator for their assigned families and ensures access to Early Steps supports and services within the community, and enhances the caregiving capabilities of the child's family and/or caregiver. Responsibility to work within the guidelines and policies of the Early Steps Program and Johns Hopkins All Children's Hospital.

Requirements

  • Bachelor's degree in psychology, social work, health education, interdisciplinary sociology, early childhood, child development, or special education, or an equivalent degree.
  • Enrollment in or eligible to enroll in the Florida Medicaid system as a Service Coordinator, within 6 months of employment.
  • 1 year (1600 hours) of hands-on professional experience with young children and families in a teaching, case management or counseling role, or with individuals with special needs and/or developmental delays of any age in a case management role; time spent in a practicum or internship (up to 400 hours) may apply toward the 1600 hours of hands-on experience.
  • Ability to travel to various work locations is required.

Responsibilities

  • Knowledgeable of the mission, policies and procedures related to Early Steps and Johns Hopkins All Children's Hospital (JHACH) and applies this knowledge to case planning for individual families.
  • Utilizes communication skills in all interactions with families, coworkers and the community, demonstrating openness and respect.
  • Is part of a team working together to address the needs, concerns, and priorities of families.
  • Knowledgeable about community resources/services to provide families access and referrals.
  • Adheres to Federal and State timelines and guidelines in order to meet program requirements.
  • Documents activities to comply with department, hospital, and State and Federal guidelines.
  • Creates and maintains accurate data records, to meet all productivity requirements.
  • Familiar with and follows HIPAA guidelines to protect Personal Health Information (PHI).
