About The Position

The Service Coordinator at Abode Services is responsible for providing housing services to individuals experiencing homelessness in Solano County. This role involves offering a variety of services aimed at increasing housing stability, including counseling, referrals to financial and legal services, and developing individualized housing plans. The Service Coordinator also ensures that participants are connected to necessary supportive services, working closely with other social service partners to enhance health and housing outcomes.

Requirements

  • Bachelor's degree in Psychology, Human Services, Social Work, Sociology, or a related field, or equivalent field experience.
  • 2 years of case management experience providing services to homeless or low-income individuals and/or families.
  • Valid California Driver's License and proof of current auto insurance with a clean DMV record.
  • Ability to work a flexible schedule, including evenings and weekends as needed.

Responsibilities

  • Provide comprehensive housing services to participants, including direct interaction with landlords.
  • Coordinate services for program participants to ensure access to essential services that improve health and housing outcomes.
  • Assist residents in retaining housing and maximizing independence by linking them to community services such as crisis intervention and employment assistance.
  • Create and maintain individual service plans and needs assessments for each household.
  • Meet with households in person as frequently as needed, either in the community or at home.
  • Attend program meetings with internal and external partners to coordinate services and ensure quality delivery.
  • Maintain client files and documentation in a timely manner using BIRP format.
  • Communicate closely with team members to improve systems and coordinate work.
  • Attend staff meetings and other agency functions as required.
