About The Position

The Social Service Coordinator plays a key role in supporting the well-being of our residents by identifying their needs and coordinating appropriate services. This position involves building relationships with community service providers, developing health and wellness programs, and advocating for residents. It is an immediate opening for a full-time, 40 hours a week, Social Service Coordinator split between 2 buildings; 20 hours at Crescent Manor, a 45-unit older adult residential community and 20 hours at Mesquite Gardens, a 48-unit older adult residential community located in Mesquite, TX.

Requirements

  • Strong understanding of available supportive services and community resources for older adults.
  • Proven ability to advocate for residents' needs and resolve issues.
  • Commitment to maintaining high ethical standards and client confidentiality.
  • Ability to work effectively with residents, staff, community partners, and service providers.
  • Capable of working independently and with minimal supervision.
  • Excellent verbal and written communication skills.
  • Strong organizational and time management skills, ensuring timely completion of tasks and resident needs.
  • Proficiency in MS Office Suite, including Word, Excel, PowerPoint, and Outlook.

Nice To Haves

  • Bachelor's degree in Gerontology, Human Services, Social Work, Psychology, Sociology, or a related field.
  • Experience working with diverse groups of seniors and disabled individuals.

Responsibilities

  • Evaluate residents' needs to identify services that can enhance their quality of life and promote independence.
  • Establish and maintain strong connections with local service providers, facilitating access to social, health, and wellness resources.
  • Create and sustain educational health and wellness programs tailored to the needs of the residents.
  • Develop and maintain a comprehensive directory of community resources and services available to residents.
  • Act as a resident advocate, empowering residents to advocate for themselves and liaising between residents and community services.
  • Build and maintain positive relationships with community agencies, networking with service providers, and identifying new services for residents.
  • Foster effective communication between residents and service providers, ensuring an open and supportive environment.
  • Serve as an advocate and liaison with management, other departments within the housing facility, and external agencies providing social services, health benefits, training, and more.
  • Provide information, referrals, and support for residents in need of crisis intervention and conflict resolution.
  • Track and monitor resident referrals to external agencies, ensuring follow-up and service delivery.
  • Maintain accurate and up-to-date records, including daily and monthly reports, to document service coordination and resident interactions.
  • Perform other tasks as assigned to support the mission and goals of RHF.
