About The Position

In this role, you will serve as a key resource for residents in Independent Living, helping them navigate services, maintain independence, and remain engaged in their community. You will collaborate with families, property management, internal departments, and external providers to ensure residents receive coordinated, person-centered support.

Requirements

  • Experience and genuine interest in working with older adults, including those with physical, cognitive, or mental health challenges
  • Strong communication, active listening, and conflict-resolution skills
  • Ability to work independently while collaborating as part of a team
  • Strong organizational, documentation, and time-management skills
  • Proficiency with Microsoft Office and electronic documentation systems
  • Valid driver’s license and reliable transportation to travel between locations

Nice To Haves

  • Bachelor’s degree in Social Work, Psychology, Sociology, or a related field or a bachelor’s degree in any field with at least one year of experience as a social worker, case manager, or care coordinator
  • Experience working in public or private social service agencies
  • Knowledge of community resources, entitlement programs, and benefits such as Medicare, Medicaid, Social Security, SNAP, and aging services
  • Experience working with diverse populations; bilingual skills (especially Asian languages) strongly preferred
  • Familiarity with HUD Service Coordinator programs and supportive housing models

  • Understanding of aging, behavioral health, crisis intervention, and trauma-informed, culturally competent service delivery
  • Ability to apply person-centered and strengths-based approaches that promote independence
  • Strong assessment, advocacy, negotiation, and documentation skills
  • Ability to respond effectively to challenging or unpredictable situations with professionalism and confidentiality
  • Demonstrated respect for diversity, inclusion, and equity in service delivery

Responsibilities

  • Meet with prospective and current residents to assess needs and support independent living
  • Provide ongoing check-ins, non-medical assessments, and service coordination
  • Assist new residents with move-in orientation, tenancy expectations, and community introductions
  • Maintain accurate resident files, service plans, documentation, and referrals
  • Coordinate educational, health, wellness, and social programs; arrange speakers and encourage participation
  • Support Resident Council activities and monthly resident meetings
  • Provide information and assistance with applications for benefits and community services
  • Support residents experiencing mental health or cognitive challenges by connecting them with appropriate resources
  • Participate in discharge planning and continuity-of-care discussions
  • Collaborate with Episcopal Homes departments to address resident needs
  • Complete required Service Coordinator training and continuing education (CEUs)
  • Perform other related duties as assigned
