We are seeking a Service Coordinator in Grand Rapids, MI at Breton Village Green who is compassionate, enjoys new challenges, and has a passion for working with people. The Service Coordinator works within HUD assisted multifamily housing to help elderly residents and persons with disabilities maintain their independence, housing stability, and quality of life. The Service Coordinator services as a link between residents and community-based services, ensuring access to resources that support health, wellness, financial stability, and daily living. This role involves assessing resident needs, developing individualized service plans, providing case management and advocacy, and coordinating supportive services from local agencies. The Service Coordinator also educates residents on available benefits and programs, promotes wellness and self-sufficiency and facilitates educational programming that builds community engagement within the property. The Service Coordinator works in conjunction with the site management and other members of the community’s management team.

Requirements

  • Working knowledge of community services in the area
  • Knowledge of eligibility requirements and procedures for applying for federal and state programs
  • Strong advocacy abilities
  • Integrity
  • Communication skills
  • Bachelors degree in social work, sociology, psychology, or related field
  • Minimum two years’ experience in human services
