The Service Coordinator assists the families of children eligible for early intervention access supports and services. The Service Coordinator ensures families receive the rights, procedural safeguards, supports and services that are authorized to be provided under Virginia’s Early Intervention program. Reports to Local System Manager/Program Manager.

Requirements

  • Under graduate degree in a relevant field, including human services, early childhood growth and development, early childhood education/special education, psychology, child and family studies, human development, interdisciplinary studies, social work, counseling, nursing, allied health [rehabilitation counseling, recreation therapy, Occupational therapy, Physical Therapy, Speech/Language Pathology], public health, education of the deaf or hard of hearing, education of the visually impaired or other related fields  specified in Table A, Virginia’s Part C Practitioner Qualifications and Responsibilities.
  • Associates degree in a related field (OTA, PTA,nursing, human services)
  • High school diploma, GED, or undergraduate degree in an unrelated filed , plus three years’ direct clinical experience in a human service field, including but not limited to human development, community systems and resources, and early child hood and family systems.
  • Must be certified as an Early Intervention Service Coordinator or Willing to obtain VA Early Intervention within the first two week of employment.

Responsibilities

  • Contact families to schedule initial intake and screening using the Ages and Stages Questionnaire.
  • Present completed Ages and Stages Questionnaire information to eligibility team.
  • Notify families of eligibility status.
  • Schedule assessment with family and notify assessment team.
  • Schedule and coordinate Individual Family Service Plan (IFSP) meeting upon completion of assessment.
  • Complete IFSP with family and assist them in developing outcomes for their child.
  • Obtain physician certification for services from child’s primary physician.
  • Notify service providers when family selects them for services.
  • Notify service providers of completed IFSP in the electronic client record.
  • Notifies finance of signed IFSP.
  • Continuous communications with providers and families to monitor status of case.
  • Facilitate transition to other services, i.e. Part B, daycare, preschool, when appropriate.
  • Collaborate with local schools to set up a transition meeting in an effort to ensure a seamless transition.
  • Complete all Part C paperwork required for children eligible for service.
  • Assist families in identifying resources and supports they may need.
  • Assist families in accessing resources and supports.
  • Maintain client record and ensure compliance with Part C regulations.
