The Service Coordinator is primarily responsible for the facilitation of child and adult-centered services and supports. This position requires the coordination of service delivery identification of resources in response to family/client needs. The Service Coordinator will provide Service Coordination services in various areas including; Service Coordination (EI), Part C Services, Children's Extensive Support (CES), Home Community Based Programs for the Developmentally Disabled (HCBS-DD), Supported Living Services, both Waiver and State (SLS), Service Coordination Only, etc. In addition, this individual will provide direct monitoring of contracted professionals. The Service Coordinator is responsible for resource allocations in each of the assigned programs and to work toward the goals of the individuals in service and the agency in a person-centered environment.
