Position Summary: The Vocational Rehab Service Coordinator is responsible for cultivating and managing new and existing community relationships with referral sources such as community agencies and programs to promote and market the services/programs of Addus by conducting community educational presentations and attending meetings. Requirements

• Must have high school diploma or equivalent.
• Must have three to five years of recent experience working in a healthcare environment or related field.
• Excellent organizational skills
• Understanding of basic medical terminology.
• Interpersonal, organizational and communication skills.
• Computer skills including but not limited to Microsoft Word, Microsoft Excel and Scheduling program.
• Willingness to travel between facilities and throughout metropolitan area.
• Must have reliable transportation, valid driver license, and state required insurance.

Nice To Haves

• Preferred additional education such as a diploma from an accredited vocational school or college.

Responsibilities

• Maintains relationships with Vocational Rehab payers
• Reviews of new referrals and coordinated with all job coaches for assignment
• Sees clients in the field for job coaching hours
• Assists in billing for job coaching hours.
• Coordinates and backs up transportation arrangements for day program clients
• Ensures all notes are collected for client visits
• Attends monthly provider meetings on behalf of the agency.
• Attends monthly case management meetings for clients
• Performs other duties as assigned.
