The organization is looking for a self-motivated, empathetic individual to provide excellent services to seniors living in Birmingham Towers. This role involves performing service coordinator duties such as resident assessments, problem-solving, and crisis intervention, utilizing knowledge of available resources. The individual will use interpersonal skills to advocate on behalf of residents and coordinate educational, informative, and stimulating programs for them, all while maintaining confidentiality. This position serves as a crucial link between residents and essential services, enabling them to age-in-place with dignity. Strong multi-tasking and time management skills are essential for this job.

Requirements

Bachelor’s Degree or Higher in Human Services Field
Driver’s License

Responsibilities

Conduct initial and ongoing functional assessments to identify residents' needs.
Help them apply for programs like SNAP, Medicaid, and utility assistance.
Establish and maintain relationships with local healthcare providers, home health agencies, and transportation services.
Organize wellness clinics, financial literacy workshops, and social events to combat senior isolation.
Provide immediate support and referral services during resident emergencies or transitions (e.g., hospital discharges or move-ins).
Maintain accurate, confidential resident files in accordance with HUD (or relevant agency) guidelines and privacy laws.
Assist in resolving conflicts between residents or between residents and management to maintain a peaceful living environment
