About The Position

Provides direction and support to service coordinators to increase service excellence for case management/service coordination activities. Maintains a caseload, acts as the primary Service Coordinator for assigned families to ensure access to Early Steps supports and services within the community, and enhances the caregiving capabilities of the child's family and/or caregiver. Responsibility to work within the guidelines and policies of the Early Steps program JHACH.

Requirements

  • Bachelor's degree with an emphasis in psychology, social work, health education, interdisciplinary sociology, early childhood, child development, or special education, or an equivalent degree
  • 3 years of professional experience as an IDEA/Part C Service Coordinator
  • Enrollment in the Florida Medicaid system as a Service Coordinator
  • Ability to travel to various work locations is required

Responsibilities

  • Monitors assigned service coordinators' compliance with federal, state and local guidelines in the implementation of the Early Steps program
  • Supports service delivery and quality to ensure families satisfaction
  • Knowledgeable about Early Steps policies and procedures to ensure assigned service coordinators are in compliance
  • Knowledgeable about documentation and billing requirements to ensure assigned service coordinators are in compliance
  • Participates in community activities/meetings to represent Early Steps and family centered nature of the program
  • Performs as Primary Service Coordinator to meet the needs and program requirements for select families
  • Knowledgeable of the mission, policies and procedures related to Early Steps and Johns Hopkins All Children's Hospital (JHACH) and applies this knowledge to case planning for individual families
  • Utilizes communication skills in all interactions with families, coworkers and the community, demonstrating openness and respect
  • Functions as part of a team working together to address the needs, concerns, and priorities of families
  • Knowledgeable about community resources/services to provide families access and referrals
  • Adheres to federal and state timelines and guidelines in order to meet program requirements
  • Documents activities to comply with department, hospital, and state and federal guidelines
  • Creates and maintains accurate data records, to meet all productivity requirements
  • Familiar with and adheres to HIPAA guidelines to protect Personal Health Information (PHI)
