About The Position

The Service Coordinator I at National Church Residences is responsible for coordinating programs and services to help participants maintain a good quality of life and age in place. This role operates under the supervision of the Property Manager/Staffing Manager/Program Manager and adheres to the organization's policies and procedures, as well as applicable federal and state regulations. The Service Coordinator prioritizes a person-centric model of service delivery, ensuring that eligible individuals receive the necessary support and resources.

Requirements

  • High School Diploma or GED required.
  • Bachelor’s degree in social work or a related field preferred.
  • Two or more years of experience in social service delivery with elderly or disabled populations.
  • Demonstrated working knowledge of supportive services and resources in the area served.
  • Good communication, comprehension, and interpersonal skills.
  • Working knowledge of Windows, Microsoft Office Suite, and the World Wide Web.

Responsibilities

  • Coordinate programs and activities for eligible individuals on a group basis.
  • Serve as a liaison to community agencies and network with service providers.
  • Monitor ongoing provision of services from community agencies and document participant use.
  • Develop and maintain a Resource Directory of local service providers.
  • Organize educational events related to health care and agency support.
  • Identify partners to provide health services and screenings to participants at home.
  • Assist participants in building informal support networks.
  • Engage volunteers within the community as appropriate.
  • Coordinate services for participants on an individual basis through assessments and monitoring.
  • Conduct telephonic and/or in-person participant assessments.
  • Collaborate with care and service colleagues to achieve participant goals.
  • Provide quality customer service to all eligible individuals.
  • Assist participants in acquiring community services to address social determinants of health.
  • Adhere to follow-up and monitoring schedules as per participant preferences.
  • Complete timely electronic and written documentation of individual interactions and services.
  • Participate in implementation of grants or special programs that benefit participants.
  • Build rapport with participants while maintaining professional boundaries.
  • Respond to all communications in a timely and professional manner.
