Service Coordinator - Early Intervention
About The Position
ACRC is seeking an Early Intervention Service Coordinator located in the Roseville Office. The Early Intervention Service Coordinator will have cases anywhere within the 10 county geographic area, and will provide case management and advocacy for children from birth to three years of age with a developmental delay, established risk condition, or who are at high risk of having a developmental disability. Responsibilities include conducting psychosocial and developmental screening assessments in the home environment; developing individual family service plans (IFSP) with families and collaborative agencies; coordinating services for infants and their families; determining ongoing eligibility of clients at age 3; completing all required forms, documentation, and reports according to regulations and ACRC policies and procedures; rotating officer-of-the-day duties with other Service Coordinators; assisting co-workers with special projects, unique problems, vacation, and sick relief as required; providing emergency on-call services when required.
Requirements
- Applicants must possess a Master’s or a Bachelor’s degree in Social Work or a related field
- Demonstrate excellent written, verbal, and interpersonal communications skills
- Have strong time management and organization skills
- Possess a valid California driver license and automobile liability insurance
Nice To Haves
- One year of professional experience in social work or a related field (preferably in the field of developmental disabilities) is preferred
