About The Position

ACRC is seeking an Early Intervention Service Coordinator located in the Sacramento Office. The Early Intervention Service Coordinator will have cases anywhere within the 10-county geographic area and will provide case management and advocacy for children from birth to three years of age with a developmental delay, established risk condition, or who are at high risk of having a developmental disability. Responsibilities include conducting psychosocial and developmental screening assessments in the home environment; developing individual family service plans (IFSP) with families and collaborative agencies; coordinating services for infants and their families; determining ongoing eligibility of clients at age 3; completing all required forms, documentation, and reports according to regulations and ACRC policies and procedures; rotating officer-of-the-day duties with other Service Coordinators; assisting co-workers with special projects, unique problems, vacation, and sick relief as required; providing emergency on-call services when required.

Requirements

Applicants must possess a Master’s or a Bachelor’s degree in Social Work or a related field
Demonstrate excellent written, verbal, and interpersonal communications skills
Have strong time management and organization skills
Possess a valid California driver license and automobile liability insurance

Nice To Haves

One year of professional experience in social work or a related field (preferably in the field of developmental disabilities) is preferred
