About the position

At Delaware County Board of Developmental Disabilities (DCBDD), we understand that life can be unexpected and we are here to help. We are committed to meeting people where they are and working together to develop a plan that supports identified needs and goals for the future. DCBDD provides support to people with developmental disabilities through two core areas: Early Intervention and Service and Support Administration. Individuals ages three (3) and above who are eligible for DCBDD services and have ongoing Medicaid waiver needs are assigned a Service and Support Administrator, or SSA. The SSA meets with each individual and their team to discuss what is important to them and their goals for the future. This information is then used to develop an outcome based person-centered plan, known as the OISP. The SSA serves as the primary point of coordination to develop and implement the OISP. The SSA works closely with each individual, family, and team of support to ensure the highest quality of service delivery. Supports can include assistance with daily living needs, therapeutic needs, learning new skills, connection to resources and social groups in the community, as well as support to find and maintain a job. Some supports may also include adaptations or environmental modifications that increase independence and remove barriers to experiencing a personally fulfilling life.

Responsibilities

  • Assist individuals to be self-directed in accessing all needed/requested services.
  • Integrate resources directed toward meeting the needs of individuals.
  • Ensure individuals receive and keep the services and/or supports to enable them to meet their goals.
  • Work to ensure the rights of individuals in accordance with federal, state, and local statutes and regulations.
  • Remove barriers to improving the individual's quality of life.
  • Engage individuals/families in developing their Ohio Individual Service Plan (OISP).
  • Conduct ongoing assessments of current needs including likes, dislikes, potential outcomes, skill development, health and safety considerations.
  • Monitor authorized services and review individuals' progress against the proposed or intended outcome of the OISP.
  • Facilitate communication with the team and address needs identified in MUI/UI and Quality Assurance reports.
  • Amend OISP as needed based on individual or team concerns.
  • Complete live targeted case management (TCM) notes to document all services delivered and service billing.
  • Oversee initial waiver enrollment process, annual waiver redetermination process, and waiver dis-enrollment process.
  • Facilitate tours, interviews, and reviews of potential provider credentials for individuals and legal guardians.
  • Identify funding sources for supports identified in the OISP.
  • Establish budgets for services based upon the individual's assessed needs.
  • Report major unusual incidents including alleged abuse/neglect.
  • Provide support to Service and Support Administrators through initial orientation, training, and technical assistance.
  • Adhere to all ORC, OAC, and DCBDD policies and procedures.

Requirements

  • Bachelor's Degree in social work, counseling, education, health and human services, law enforcement, behavioral science, or a related field.
  • Master's degree and two (2) years' experience working with individuals with developmental disabilities preferred.
  • Ability to obtain and maintain DODD Service and Support Administration certification per OAC 5123-5-02.
  • Must possess a valid State of Ohio Driver's License and acceptable driving record.
