About The Position

The Sepsis Nurse Navigator at Providence Mission Hospital plays a crucial role in providing culturally competent care to patients diagnosed with sepsis, particularly those with Limited English Proficiency (LEP). This position involves collaborating with the care team to implement program goals, offering education and support to patients and their families, and facilitating discharge planning and follow-up care. The Navigator also engages with community health resources to promote overall wellness and health education, ensuring that services are tailored to meet the unique needs of individuals and families.

Requirements

  • Bachelor's Degree in a health-related field, preferably Nursing or Public Health Nursing.
  • California Registered Nurse License upon hire.
  • National Provider BLS certification from the American Heart Association upon hire.
  • Minimum of 3 years of acute care experience.
  • At least 1 year of experience working with patients with Limited English Proficiency (LEP).

Nice To Haves

  • Coursework or training in case management.
  • Experience working with diverse cultures and education levels.
  • Experience in Public Health Nursing, Home Health Nursing, or Community Health settings.
  • Experience collaborating with community groups, educational institutions, or faith-based organizations.
  • Some supervisory experience.

Responsibilities

  • Facilitate care for patients with Limited English Proficiency (LEP) diagnosed with sepsis.
  • Provide education and support to LEP patients and their families.
  • Enhance communication between patients and the care team.
  • Assist in discharge planning and follow-up support for up to 30 days post-discharge.
  • Collaborate with community health/nurse navigators and organizations to promote health and wellness.
  • Conduct health education and advocacy, including one-on-one teaching and support groups.
  • Develop and implement programs to improve the quality of life for patients in the community.
  • Cultivate positive working relationships with hospital staff and community members.
