About The Position

The Senior Value Based Care Clinical Program Manager at HealthPartners will lead the development, implementation, and evaluation of programs aimed at enhancing patient care and outcomes within a value-based care framework. This role emphasizes collaboration with interdisciplinary teams to leverage data insights and drive innovative solutions across various care settings, including acute, post-acute, and ambulatory care.

Requirements

  • Bachelor's degree in a healthcare-related discipline.
  • Seven years of population health or value-based care clinical leadership experience.
  • Strong program/care model evaluation experience.
  • Understanding of value-based care models, quality measurement, and reimbursement structures.
  • Strong analytical skills with experience in healthcare data analysis.
  • Excellent communication and people skills, capable of presenting to diverse audiences.
  • Ability to work collaboratively in a fast-paced environment.
  • Demonstrated capability to manage multiple projects and activities effectively.
  • Knowledge of budgetary responsibilities and risk management.

Nice To Haves

  • Master's degree in social work, nursing, or related healthcare discipline.
  • 10 years of hospital or healthcare-related leadership experience.
  • Valid and current discipline license in Minnesota or Wisconsin.
  • Project management and business planning experience in a healthcare setting.
  • Experience implementing value-based care clinical programs for multi-hospital systems.

Responsibilities

  • Design and implement value-based care clinical programs that align with organizational strategies and goals.
  • Oversee the development, implementation, and evaluation of clinical programs to enhance patient care and outcomes.
  • Utilize data analytics to identify intervention opportunities and assess program effectiveness.
  • Foster a culture of innovation by implementing new technologies and processes that enhance care delivery.
  • Build collaborative partnerships and lead cross-functional teams to execute value-based care initiatives.
  • Develop training materials and conduct workshops for clinical teams to enhance understanding of value-based care principles.
  • Establish metrics and dashboards to monitor program effectiveness and impact of initiatives.
  • Participate in various committees, task forces, and quality improvement teams as needed.
