Senior Transitions (Long Term Care/Post Acute Care) Physician – St. Cloud, MN
About The Position
Join an expanding geriatric focused service line that provides mobile primary care services to multiple sub-acute, assisted living, and long-term care locations within a 60-mile radius of St. Cloud. This position is eligible for hiring incentives to include sign-on bonus and relocation assistance. Summary In collaboration with our 4 current physicians and 2 advanced practice providers, you will support our advanced practice providers in their onsite geriatric primary care practice for our long-term care patients by completing the physician required regulatory visits, rounding in our subacute settings, and help support our APPs with MD required services in our assisted living facilities. This role is supported by nursing and clinic assistants. This position will provide you with flexibility and independence in your practice while feeling supported by a care team that enables our doctors to have the time to provide outstanding care to our patients. Provide comprehensive evaluation, consultation, and treatment services to geriatric patients in a variety of settings.
Requirements
- BE/BC in FM or IM
- Eligibility for Minnesota medical licensure
