About The Position

This HUD-VASH Senior Social Worker provides onsite case management to Veterans residing at these two Project Based locations: King 1101 Apartments (1101 W. Martin Luther King 90037) and Metro at Western (3671 S. Western Ave. 90018) Provides case management services to a special patient population that demonstrates highly complex psychosocial, health or mental health problems. Provides crisis intervention as needed and have advanced knowledge and experience in the use of medical and mental health diagnoses, disabilities, and treatment procedures, including acute, chronic, and traumatic illnesses and injuries, common medication and their effects, side effects, and medical terminology. Coordinates and provide information and referrals to VA programs in their provision of case management services. Independently work with patients and families who are experiencing a variety of psychiatric, medical and social problems, utilizing individual, group and family counseling and therapy skills. Provides consultation services to other VA staff regarding the psychosocial needs of homeless Veterans and the impact of psychosocial problems on health care and adherence to treatment plans. Documents in the patient's medical record in accordance with medical center policies and the requirements of The Joint Commission (TJC) and other regulatory bodies. CPRS notes are timely and accurate. Complies with criminal conflict of interest laws and Executive Branch Standards of conduct to avoid conflicts of interest while serving as a Senior social worker for VA's Health Care for Homeless Veterans Programs. Responsible for tracking census of veterans within the program and ensure that Veteran data is captured in HOMES. Work as a field instructor with MSW student interns or clinical supervisor for unlicensed VA staff, when requested. Must be able to operate a government furnished vehicle for the purposes of assisting Veterans with obtaining and maintaining housing.
