About The Position

The Veterans Directed Care (VDC) and Community Adult Day Health Care (CADHC) Program Coordinator is a social worker who is responsible for the professional and administrative management of the Veteran Directed Care (VDC) and Community Adult Day Health Care (CADHC) for the Columbia VA Healthcare System. The Social Worker (Program Coordinator) duties and responsibilities include, but are not limited to the following:: 1) The program coordinator oversees the daily operation of the program, develops policies and procedures for program operation, and prepares reports and statistics for the GEC Service Line Chief, Medical Center, VISN, or national level staff. 2) Provides input on the program's budget, as applicable, and develops and monitors staff compliance with practice, standards, and guidelines on documentation, workload, data entry, ethical practice, and service delivery. 3) Oversees program operations and evaluation, identifies areas of improvement, gathers relevant data, assesses data, develops and implements ideas for improvement, and evaluates the efficacy of the program improvement efforts implemented. 4) Receiving CADHC consults and managing referrals and the initiation of services within the SWS Scope of Practice. Use the Consult Tracking Manager to monitor and reauthorize expiring CADHC consults proactively. 5) Determining eligibility for services via the Case Mix Tool and working with Veterans, families, and the CADHC Centers to coordinate the desired placement. The incumbent will complete appropriate documents, reviews, and follow up on Veterans' complaints regarding the applicable services. 6) Timely submission/release of clinical referrals to GEC administrative staff for the creation of appropriate financial authorizations. Work with GEC staff to ensure a timely and accurate completion of the financial authorizations for the CADHC Program. 7) Perform work related to the review, monitoring, analysis, planning, and assessment of the quality of care provided to Veterans enrolled in this service. 8) Performs other program duties as outlined in the VHA Office of Integrated Veteran Care (IVC) Field Guidebook, Chapter 8: Specialty Programs section. 9) The program coordinator oversees the daily operation of the program, develops policies and procedures for program operation, and prepares reports and statistics for the GEC Service Line Chief, Medical Center, and VISN-level staff 10) Negotiate and facilitate the basic ordering agreements/contracts for the VDC as needed. This ensures the vendor's adherence to applicable VA standards and guidelines required to promote the health, safety, welfare, and satisfaction of the Veteran receiving community services. 11) Performs other related duties as assigned.
