About The Position
The Senior Social Worker will provide essential services within the Homeless Patient Aligned Care Team (HPACT) clinic and serve as a liaison for an on-campus shelter in the Community Engagement and Reintegration Service (CERS) at the West Los Angeles Healthcare Center. This role focuses on addressing the complex psychosocial issues faced by Veterans, including homelessness, mental health challenges, and social support deficiencies, while collaborating with interdisciplinary teams to enhance treatment outcomes and facilitate community reintegration.
