About The Position

The Senior Social Worker position at the Department of Veterans Affairs involves providing direct services to Veterans, their families, and caregivers at a Community Based Outpatient Clinic. The role includes conducting psychosocial assessments, participating in treatment planning, and providing consultation and education on community resources and VA benefits. The Senior Social Worker also offers leadership and training to social work staff, ensuring high-quality care and equity of access to services for Veterans.

Requirements

  • Must be a U.S. Citizen.
  • Proficient in written and spoken English.
  • Master's degree in social work from an accredited school.
  • Licensed or certified by a state at the advanced practice level.
  • At least two years of experience post-advanced practice clinical licensure, with one year equivalent to GS-11 level.

Nice To Haves

  • Experience in medical social work, either outpatient or inpatient.
  • Certification or post-masters training from a nationally recognized professional organization.

Responsibilities

  • Conduct psychosocial assessments to determine the needs of veterans and family members.
  • Participate in treatment planning with other disciplines.
  • Engage in discharge planning and ensure timely execution of discharge plans.
  • Provide consultation and education to veterans and families on community resources, advance directives, and VA benefits.
  • Act as a consultant to other team members and provide guidance on social work practice and VA policy.
  • Serve as a liaison between VA and community resources, including marketing and public relations duties as needed.
  • Provide a full range of social work services including case work, individual, family, and group counseling, and case management.
  • Facilitate patient/family support groups and wellness/prevention education as appropriate.
  • Supervise the practice of social work students or less experienced social workers as needed.
  • Coordinate treatment and communicate with care lines to facilitate continuity of care for veterans.
  • Maintain data and statistical compilations to comply with relevant policies and procedures.
  • Document social work interventions and activities in patients' clinical records.
