About The Position

Anoka County Community Social Services and Behavioral Health (CSS/BH) currently has one full-time Senior Social Worker position available. This role is responsible for providing assessments and early intervention for individuals referred to the Behavioral Health Intake unit. The unit serves children and families by offering early intervention and resource navigation for up to 180 days. Using a person-centered approach, the primary goal is to meet individuals where they are and assess their needs. During this period, social workers coordinate access to appropriate county and community services tailored to each individual's or family's mental health and social service needs. When necessary, individuals are transferred to ongoing service teams for continued support. Behavioral Health Intake services are open to all Anoka County residents, regardless of age, income, diagnosis, or insurance status. Referrals are accepted from a wide range of sources, including hospitals, treatment facilities, outpatient clinics, schools, and law enforcement agencies. Requirements

  • Bachelor’s degree in social work or a closely related field.
  • Four years (2,080 annual hours) of job-related experience working directly with children with SED or mental health diagnosis/es.
  • In lieu of the required education, an equivalent combination of education and experience may be substituted on a year-for-year basis.
  • Valid driver’s license and vehicle available for business use.

Nice To Haves

  • Master’s degree in social work or a closely related field.
  • Current mental health professional licensure or the ability to obtain within six months of hire.
  • Five years or more (2,080 annual hours) of direct work experience working directly with children with SED or mental health diagnosis/es.
  • Independent Licensure as a mental health professional.
  • Experience working within SSIS database.
  • Strong verbal and written communication skills.
  • Skilled in problem-solving, critical thinking and effective time management.

Responsibilities

  • Screen, assess, plan, refer, and provide early intervention case management to children and families referred to the Behavioral Health Intake unit for services.
  • Conduct outreach, and monitor services designed to meet the individualized mental health needs of children.
  • Support families as they strive to understand and meet the behavioral health needs of their child.
  • Conduct face-to-face visits with child, parent, and/or primary caregiver in the community or appropriate milieu.
  • Complete functional assessments, individual family community support plans and/or other plans with the youth/family that outline the services they are eligible to receive.
  • Help children/families access county programs and community-based services such as home-based therapy, individual therapy, family therapy, IOP, partial hospitalization programs, crisis stabilization services, and medication management, that help meet their service needs.
  • Assist children/families with the process of obtaining least restrictive mental health care in the community, and/or obtaining higher level of care as needed.
  • Generate and share resource and referral information on the continuum of services for children and families.
  • Facilitate clinical screenings and/or Multidisciplinary Team (MDT) meetings and case transfers as needed.
  • Use and navigate SSIS and OnBase for timely completion of needed paperwork, case notes, assessment documents, and electronic files.
  • Participate in individual and group clinical supervision in compliance with state statute.
  • Attend and participate in continuing education, training, or in service offerings required to maintain license in good standing.
  • Participate in unit and/or county meetings related to service provision as requested or required.
