About The Position

The Senior Quality Associate is responsible for supporting population health initiatives designed to improve the quality of care provided to Stamford Health Medical Group patients. Reporting to the Manager of Quality Services (Manager of Quality), the Senior Quality Associate provides support with managing and optimizing participation in programs including the Merit-Based Incentive Payment System (MIPS), Patient-Centered Medical Home (PCMH), and other value-based payment programs. Required to participate in monthly payor meetings, identify and manage critical care gaps, and will lead physician outreach as needed. The Senior Quality Associate is responsible for assisting with supervision over the day-to-day tasks of the Quality Associates. Under the direction of the Manager of Quality, the Senior Quality Associate supports the implementation and oversight of the population health platform and initiatives. Additionally, the Senior Quality Associate will provide support with training and report validation for the SHMG Quality Program. Finally, the Senior Quality Associate will also have limited pre-visit planning responsibilities for assigned primary care and OB/GYN practices.

Requirements

  • Bachelor’s degree required.
  • Two or more years of related professional experience in a medical group setting is strongly preferred.
  • Experience with electronic health records (EHRs) is required. Experience with Epic is preferred,
  • Working knowledge of population health programs including Merit-Based Incentive Payment System (MIPS), Patient-Centered Medical Home (PCMH) and Healthcare Effectiveness Data and Information Set (HEDIS) is preferred.
  • Proficient in Microsoft Office: Excel, Word, and PowerPoint.
  • Strong written and verbal communication skills.
  • Superior organizational and interpersonal skills.
  • Detail and process orientated with a focus on efficiency.

Nice To Haves

  • Master’s degree preferred.
  • Experience with Epic is preferred,

Responsibilities

  • Participates in monthly payor meetings and leads projects around care gap management and annual care appointments.
  • Outreaches to physicians, as needed, to communicate key preventive health measures related to their patient panel.
  • Assists with supervision of the day-to-day operational aspects of assigned projects and oversees project work completed by the Quality Associates.
  • Provides key support around the management of the SHMG Quality Program.
  • Reviews and validates reports and meets with physicians to promote education and program compliance.
  • Assists with monitoring reports to assess program success against program goals and to identify opportunities for improvement.
  • Supports the implementation of the population health platform and works with the Manager of Quality to develop workflows and best practices.
  • Assists the Manager of Quality with managing processes within the population health platform to proactively identify patients for preventive care needs and services.
  • Supports quality improvement initiatives across the medical group as assigned including patient experience improvement.
  • Provides training for providers, clinical staff and front desk staff related to quality programs including new employee orientation.
  • Supports the Quality Project Manager in reviewing and validating patient experience reporting and transparency comments.
  • Responsible for limited pre-visit planning activities for assigned primary care and OB/GYN providers. Planning activities include but are not limited to lab and imaging tracking.
  • Develops strong relationships with providers, practice managers, and medical assistants.
  • Attends and participates in quality team meetings.
  • Attends scheduled staff meetings, in service education opportunities, and participates in professional development opportunities.
  • Formulates and maintains effective working relationships with employees and other managers of the health care team.
