About The Position

The Senior Project Manager serves as a strategic partner to executive leadership, leading cross-functional initiatives that support Home Health, Hospice, and Home Care operations. This role provides high-level project oversight, ensures organizational alignment, and drives execution across multiple departments. The Senior Project Manager maintains a proactive approach, anticipates needs, and supports leaders through structured workflows, communication, and operational coordination.

  • Minimum 5 years of project management and/or senior-level executive support experience, preferably in healthcare, home health, or hospice environments.
  • Demonstrated experience coordinating multi-department initiatives and working directly with executive leadership teams.
  • Advanced proficiency in Microsoft Office (Outlook, Word, Excel, PowerPoint), project management software, workflow tools, and digital scheduling platforms.
  • Strong writing, documentation, and communication skills, with the ability to prepare polished executive-level reports, updates, and project summaries.
  • Proven track record of managing multiple projects simultaneously in a fast-paced, high-visibility environment.

Responsibilities

  • Lead and manage complex, enterprise-level initiatives spanning Home Health and Hospice, ensuring alignment with organizational goals.
  • Serve as project manager for senior leadership—including the CEO, VP of Operations, CFO, VP of Sales, and other executives—overseeing coordination, scheduling, communication, and follow-through.
  • Develop and distribute comprehensive project scopes, ensuring stakeholders understand objectives, timelines, deliverables, and roles.
  • Ensure all required documentation is collected from directors and submitted prior to project initiation.
  • Maintain a highly proactive project management approach, consistently following up on action items, milestones, and timelines to ensure on-time completion.
  • Coordinate and facilitate internal meetings; prepare agendas, document minutes, track action items, and ensure appropriate team members are included.
  • Manage executive-level correspondence and mail while maintaining strict confidentiality and professionalism.
  • Support executive leaders with follow-ups, project communication, progress documentation, and cross-departmental coordination.
  • Utilize quantifiable metrics to track project progress, identify risks, and communicate outcomes to senior leadership.
  • Build and maintain strong relationships across CommuniCare facilities, serving as a liaison to support operational integration across Home Health and Hospice divisions.
  • Facilitate training documentation, standardize processes, and support workflow alignment across project teams.
  • Manage multiple high-priority initiatives simultaneously using advanced scheduling, organization, and prioritization skills.
