Our mission is to make preventive, coordinated care accessible and personal — blending high-touch engagement with digital tools that help members stay connected and supported at home. As we expand our virtual care offerings, we’re building a small, nimble Care Delivery team that integrates clinical operations, product, and outcomes under one umbrella. This role is foundational to that mission. The Role We’re looking for a Senior Product Manager, Care Delivery — a hands-on operator who will own how we activate, enroll, and engage members across both tech and human touchpoints. You’ll function as both product manager and implementer, working across our app, AI engagement tools, CRM, and EHR systems to make our care model scale seamlessly. You’ll join a lean, execution-oriented team where everyone builds, learns, and iterates quickly. This is not a “manager of managers” role — it’s a player-coach role for someone who loves designing systems and getting into the weeds to make them work.

  • Own the member journey: Design and optimize onboarding, activation, and engagement flows across app, phone, care-team touchpoints and operational workflows
  • Drive product & operational execution:
  • Define, prioritize, and implement requirements for our care platform (EHR, patient CRM, engagement tools, member portal).
  • Write and QA tickets, configure workflows, and test features directly with clinicians and coordinators.
  • Integrate tools: Connect CRM, analytics, and automation platforms to streamline operations.
  • Collaborate cross-functionally: Partner with clinical ops and data teams to turn frontline feedback into product improvements.
  • Measure outcomes: Track and report on activation, retention, and satisfaction metrics to guide iteration.
  • Champion the member and the clinician: Translate user insights into intuitive digital and offline experiences.
  • 3–6 years of product or operations experience in digital health, health tech, or SaaS-enabled services.
  • Proven ability to work cross-functionally with clinicians, data, and engineering.
  • Comfortable being an individual contributor who builds as much as they plan.
  • Strong familiarity with care delivery systems — EHRs, CRMs, engagement platforms, or equivalent tools.
  • Analytical and detail-oriented: you use data to drive prioritization and measure impact.
  • Comfortable working in ambiguity and resource-constrained environments.
