Cornerstone Rehab is seeking an experienced and motivated Senior Physical Therapist to oversee clinical responsibilities across multiple facilities. Requirements
- Valid State Physical Therapist License and Credentials
- SNF and/or Acute care experience
Responsibilities
- Provide physical therapy to adult and geriatric patients.
- Prepare documentation, progress notes, and discharge summaries.
- Establish a plan of care in accordance with patients' needs and capabilities.
- Travel to nearby centers as needed
