We are looking for a Senior Nurse Navigator to support our breast medical oncology patients in the main outpatient clinic. The Nurse Navigator will offer personalized guidance to new patients, helping them understand and navigate their treatment plan. This includes coordinating care and referrals after their initial visit. The role also involves ensuring that follow-up appointments are scheduled as patients begin chemotherapy. The Nurse Navigator works closely with physicians and clinic staff across all specialties to ensure seamless coordination of care and communicates with the entire care team on behalf of the patient.
